Contact our online GP in times of bereavementv

  1. Telephone the doctor or LAS who will visit to confirm that death has taken place
  2. Contact a funeral director
  3. Arrange to collect the doctor's Medical Certificate of the Cause of Death (usually from the GP Surgery)
  4. Take this to the local Registrars Office, (together with the deceased's Medical Card and Birth Certificate, if available). Alternatively you can register by declaration at any convenient Registrars Office but certificates will not be available as these will have to be posted to you a few days later
  5. The Registrar will normally issue a Green coloured certificate for you to give to your funeral director who will look after the necessary arrangements for the funeral. The Registrar will also issue a white notification certificate for the DSS. They will also enquire as to the number of Certified Copies you require for dealing with the deceased's finances (a fee is payable for each copy)
  1. Contact a funeral director to inform them that thier services are required
  2. Collect the Death Certificate from the hospital then follow steps 4 - 5 as above

Your funeral director will usually liaise directly with the surgery regarding the additional certification required

Our clinical team

We operate with a broad range of clinically skilled team members. This means we have prescribing nurse practitioners, prescribing pharmacists, physician associates and advanced clinical practitioners seeing and calling patients as well as our GPs. This also means we are able to help solve patient issues on the same day, using the best clinician (with our triaging system).

If there is a clinical need for a face to face consultation, perhaps for an examination, then a Practice appointment will be made with the most appropriate clinician (this may not be a GP). Some of our clinicians are based at one or more of our sites.

Get In Touch

Our standard surgery hours are Monday to Friday, however you can register online 24/7. Registration allows you to book appointments , order repeat prescriptions and provides access to a wide range of online clinical services . If you'd like any further information, please contact our practice.
